The solution

Comunicare is software solution that enables patient education, therapeutic compliance, and patient reported outcomes, therefore better quality of life, better communication with care givers, better quality of care.

The mobile application provides personalized information to patients on their care journey, especially care agenda, the description of the therapy and its possible adverse effects, and recommendations for daily life. It generates smart notifications to ensure patient compliance (e.g., recall of consultations and intake of home medication) and outcome reporting (vital signs and symptoms). Alerts can be generated and presented to the patients when reported data match predefined criteria that are evaluated in realtime thanks to the integrated and configurable rule engine.

Patient reported data enables a better evaluation of the therapy and to provide proactive care. For caregivers, a dashboard component aims to suport medical decision support. This dashboard can be integrated in the Electronic Health Record (EHR) using standard data format such HL7/FHIR or proprietary data format.

How can it help to manage the COVID-19 pandemic?

The solution has native functionality to provide self-management at home for suspected or confirmed patients with Covid-19 but who do not require hospitalization.

The mobile application allows:

  • to inform patients about the pathology and provide them with personalized recommendations validated by caregivers,
  • collect parameters and symptoms at home and facilitate monitoring for healthcare teams.

Comunicare also allows Covid-19 to be taken into account in the most at risk chronic diseases, in particular patients suffering from chronic systemic diseases of respiratory origin, such as chronic obstructive pulmonary disease (COPD).

The benefit for patient is self-management and reduction of anxiety. The benefit for caregivers and healthcare organizations is to free time and space for more serious cases while maintaining a remote follow-up for less complex cases.
